Must be filled out for all Cautions, Send-Offs (Including spectators), Injuries that may require medical treatment, Game Abandonment, etc.

Fill out and email to Tim McCurley at tmccurley@spindletopsoccer.com and copy your assignor. Reports must be filled out either on the triplicate form at the fields or electronically using above form. They must be submitted within 48 hours of the game. If you use the triplicate form turn the entire form in the box. Your Assignor will keep the Pink Copy for your club's records. The white and yellow copies will go to the Association.

•  Make sure to put the player/coaches name (First and Last), ID Numbers or Jersey Numbers, and proper caution or send-off offense on the report.
• Do not use slang terminology (ie. Retalitory Push, Dangerous Play, Kicking). Refer to the 7 & 7 Cautionable and Send-Off Offenses.
• If you send off a spectator, be sure to have them identified by someone and put their name on the report and what team they represent. If you have problems getting this information, note it on the supplemental report.
• The supplemental form MUST be filled out when there is a Send-Off (Player/Coach/Spectator). Be detailed in these reports. If you have any questions regarding filing of reports or filling them out, contact Tim McCurley or your club's assignor.
